ACA Form 1095 C Request Notice

Notice to Employees Regarding Form 1095 C Availability

Seaport Steel does not automatically provide Form 1095 C to employees. Full time employees may request a copy of their Form 1095 C for the applicable tax year at any time throughout the year.

Employees may submit requests using any of the following contact methods:

Upon receiving a request, Seaport Steel will furnish the Form 1095 C by the later of:

• January 31 of the year following the year to which the form relates, or
• 30 days after the date the request is received.

This notice will remain posted through October 15, 2026
